I am choosing to continue my participation in the Walker Voucher Program.

I understand that I will continue to receive the benefit of the 125% of Fair Market Rent (FMR).
I understand that I will need to move and relocate to a unit that is located in a census tract that is considered a court targeted area according to the 2020 census. This move is not required until my next regular re-examination, or at the end of the year. This deadline can be extended to the expiration of the current lease term or for other good cause, such as waiting until the end of a school year. I may choose to remain in my current unit; however, this would mean that I would no longer be able to participate in the Walker Voucher Program. If I remain in this unit, I will lose the higher payment standard of 125% given with the Walker Program Voucher. This change in my payment standard will not happen until the end of the year.
I understand that at the time of my move I will be eligible for some mobility assistance including application fees, moving assistance, utility connection fees, and security deposits. This mobility financial assistance will be provided by DHA. The Inclusive Communities Project, Inc. (ICP) will provide mobility counseling including search assistance. ICP can be reached at 214.658.1339 or map@inclusivecommunities.net.
I understand that when I relocate, I will need to follow the standard relocation procedures and requirements. Relocation packets to assist in searching for a unit in a court targeted area using the 125% payment standards will be available.
I understand that I am subject to all program requirements for continued assistance.

I understand and agree that:

I am requesting to transfer from the Walker Settlement Voucher Program to the Housing Choice Voucher Program.
Once I convert from a Walker Settlement Voucher to a regular Housing Choice Voucher, I will not be able to convert back.
By converting from the Walker Settlement Program to Housing Choice Voucher, I will lose the benefit of the 125% of Fair Market Rent and other benefits of the Walker Settlement Voucher.
I am subject to all program requirements for continued assistance.
